Description:

You will be part of a highly responsive and holistic service, that supports children to access, a wide range of meaningful social, leisure, sporting and vocational opportunities, that increase self-confidence, improves health & wellbeing and develops communication and social skills. You will helpprovide outcome focused, person centred support, at community hubs across Birmingham, empowering people to become active membersof their communities. You will be reasonable for insuring that citizens are supported to access meaningful opportunities and to achieve their own personal goals and ambitions, you will assist with the planning and delivery of the activity timetable, which will involve both internal and external facilitators. This role also involves providing personal care, as and when requiredUnder the direction of the Personal Assistant/Hub Coordinators you will be responsible for the below;
*Escorting and supporting individuals to the hub's day placements, and community based activities in accordance with the individual's support plan.
*Ensuring that all community based social activities are risk assessed based on each individual supported.
*Responsibility for ensuring your own personal safety and the safety of the individual in accordance with the companies policies and procedures including Health and Safety procedures, Financial procedures and Confidentiality.
*Liaising with the client, their family, carers andstaff to ensure that the service provided is person centred
*To uphold and understand the rights of the client by supporting them to make individual choices based on their personal needs and preferences.
*Report, record and monitor the client's individual progress, through observation and record keeping, reporting any concerns to the Personal Assistant/Hub Coordinator
*Followfirst aid procedures and keep records of all accidents, incidents and treatment given
*Promote the company to families and other agencies
*Ensure appropriate professional boundaries with all clients, parents and carers, staff and volunteers are maintained across the service

The ideal candidate would;
*Be able to work as part of a team
*Demonstrate an ability to motivate andencourage people with learning disabilities to participate in activities
*Show an understanding of alternative methods of communication that could be used with non-verbal service users
*To plan and deliver creative programmes to meet individual levels ofability and need
*Be able to participate in sports and other physical activities
*Demonstrate an ability to work on own initiative

Successful candidates will be willing to undertake an enhanced DBS and the applicants cost, hold a minimum of 6 monthsexperience of working with children with learning disabilities. You will demonstrate the ability to communicate effectively with colleagues, parents, carers and service users and hold good written and verbal communication skills
