Community Clinical Nurse Specialist (CNS)
Are you an experienced registered nurse who would like to be part of a friendly, energetic and growing team? An exciting opportunity hasarisen for an experienced Community Clinical Nurse to work as part of the 24 hour community nursing team, providing high qualityholistic care to patients. This particular vacancy requires an exceptional Community Clinical Nurse to be responsible for assessment, planning, implementation and evaluation of evidence based nursing to patients in their own home. You will maintain clinical competency in a range of extended skills, as required to meet the needs of patients on the caseload.
The Community Clinical Nurse Specialist (CNS) is responsible for effectively managing and coordinating a caseload of patients and families living with specialist palliative care needs. You will ensure a flexible, responsive and needs led approach to care out in the community and make clinical decisions including assessing the physical, psychosocial and spiritual needs of patients and their families living within their home including care homes using patient centred principles.
The Ideal Community Nurse will have the following:
* Registered Nurse
* Minimum of five years post registration experience
* Understanding and experience of multi-disciplinary team working
* Experience of using reflective processes as a framework to improve practice
* Experience and knowledge of palliative care
* First level degree, or willingness to undertake in palliative care or a related area
* Knowledgeof current National Palliative Care guidance and initiatives
* Full UK driving licence holder and access to a car for workBenefits include:
