Description:

We have an excellent opportunity for a Lead Support Worker based in Swindon within a Transforming Care team, supporting a customer to live a fulfilling life in their own home within the local community. The team support customers who have complex support needs, including learning disabilities, mental health issues,autism communication differences, alongside ongoing physical health conditions.

You'll work primarily with the customer in their own home in Swindon, as well as assisting them in the community. But there will likely be opportunities to work with our other customers in Swindon! You can use public transport to support them and we'll pay your expenses to cover this!

Working hours:

The support team will work 24/7 to meet the customer's needs, so you'll need take your turn with your team working on a rota which could include working days, evenings, weekends, bank holidays and waking nights. You may also be required to do sleep ins or be on call in whichan enhancement will be paid.

Day team members 7am-10pm Night workers 9pm-8am. Flexible shift times.

Permanent, Full time,37.5 hours + excellent benefit package

Sleep-In and On Call enhancements paid, 25 days annual leave (on top of bank holidays) is given as standard, rising a day a year up to 30 days. With the option to buy up to five days extra days leave.

About youYou must have an excellent working knowledge and experience in CQC Regulated services. It's important that you have worked at a Senior Support Worker level before, with experience of leading support for people with complex needs, including those with significant behaviours that challenge.

Holding a Level 3 Diploma in Adult Health & Social Care or equivalent, with a greatunderstanding and passion for promoting independence and social inclusion through person centred support.



You'll have agreat knowledge of professional boundaries, safeguarding, risk assessment and working with personalised support plans and usingPBS.

You'll need to have a valid Enhanced Check under the Disclosure and Barring Service.
