Job Details
A full-time, permanent vacancy has arisen for a Community Staff Nurse. The Community Staff Nurse will report directly to the Hospital @ Home Clinical Nurse Managers.
Key responsibilities will include but are not limited to
* Provide efficient and effective direct care Home patients.
* Provide a positive contribution to the company on a daily basis.
*Practice according to the professional code by the Nursing and Midwifery Board of Ireland and the policies and procedures of Hospital @ Home.
* Work to the highest clinical standards ensuring patient safety by adhering to Hospital @ Home clinical policies and procedures.
* Ensure implementation of care plans for each patient at the appropriate time and feedback any changes in the health of the patients to their respective Clinical Nurse Manager.
* Work as part of a team, and also on one's own initiative.
* Participate as a team member of the multi-disciplinary team in all aspects in the delivery of patient care.
* Attend team meetings as required.
* Maintain professional registration with the Nursing and Midwifery Board of Ireland and provide evidence of same annually, and in the required timeframe requested by Hospital @ Home team.
* Attend training courses and continuous medical education sessions, both in-house and at other locations, to meet and enhance the service being provided.
* Participate in the development of care plans.
* Collaborate with the Clinical Nurse Manager in preparing and implementing patient care plans.
* Participate in the identification, development and implementation of client and/or staff education programs.
* Undertake clinical audits as directed by the Clinical Nurse Manager or Senior Nurse Manager
* Review allocated visits by the Clinical Nurse Manager for any given day on duty on the evening prior to shift.
* Provide handover to community staff nurse colleagues on completion of rostered working day (if not on duty the day following).
* Prioritise the workload of the day, and organise resources as required.
* Provide current information on the health status of their allocated patient to the Clinical Nurse Manager and Hospital @ Home Community Physician.
* Collaborate with patients and the multi-disciplinary teams to facilitateappropriate continuity of care.
* Check the patient's vital signs at each visit.
* Perform Cannulation and Venepuncture when required, to the highest clinical standards.
* Access and maintain CVAD lines as per protocol
* Working across all H&WServices such as Medical Screening
The Successful candidate must have:
* Minimum 3 years post registration experience with clinical experience in medical or surgical setting
* General Nursing Qualification
* Registration with the Nursing and Midwifery Board of Ireland with NMBI pin
* Eligible to work in Ireland ie must have visa, etc
* Previous experience in a community setting
* Experience in a commercially focused environment
* Proficiency and post qualification experience in phlebotomy and IV Cannulation
* Excellent knowledge and experience administering Intravenous Therapies
* Demonstrate sound knowledgeof caring for medical/surgical patients
* A clear understanding of common computer applications.
* A clear desire and provencapability to work on your own initiative in a clinically challenging and protocol led environment.
* A proven interest in advanced nursing practices, and a high level of attention to detail.
* Ability to develop and maintain strong working relationships
* Ability to communicate effectively and professionally.
* Ability to work independently and provide regular feedback* Demonstrate alignment with hospital values
