My Client are an NHS Trust based in West Midlands and are seeking a Lead Complex Case Manager - Specialising in ABI on an initial 6 month contract basis (37.5 Hours) Paying £381 per day (Inside IR35).

Job Summary

* The post holderwill be responsible for the day-to-day operational management of the Birmingham Solihull CCG.
* The Clinical Commissioning Officer ABI will be an experienced and qualified health care professional and will take responsibility for the co-ordination of specialist assessment and treatment of clients suffering acquired brain injuries.
* They will work as an inter-disciplinary member of the Continuing Healthcare Team and in conjunction with the ABI service, working in conjunction with other providers, both statutory and independent, to meet the needs of this highly complex client group.
* The Clinical Commissioning Officer ABI willdevelop individual pathways of care, sourcing new and existing services to meet the needs of individuals with acquired brain injury. This necessarily involves leading interagency negotiation and provision for people with highly complex needs with the support of the Clinical Team Manager.
* To undertake specific designated responsibilities for management tasks within the AcquiredBrain Injury Service, including deputising for case management issues for the Clinical Team Manager.
* To contribute to the future planning of service needs liaising with service users, user groups, providers, and commissioners, to implement service improvements for people with acquired brain injuries across all aspects of the client journey in the primary and secondary health care setting.
* The post will include supporting and giving advice to clients and carers where highly complex clinical information will be discussed and where there may be significant barriers to understanding e.g. cognitive impairment, insight problems and communication difficulties.
* The post holder will be working with clients and staff in a variety of settings, in primary and secondary health care, as well as the client's home, places of work, place of leisure or preferred social settings.
* The Clinical Commissioning Officer ABI will be responsible for successful delivery of a range of activities to achieve agreed outcomes and to secure improvements to ABI clinical pathway

Experience and Knowledge

Essential

* Demonstrable experience of working in a large complex organisation.
* Experience of working within the NHS.
* Knowledge of specialist areas, Neo rehab,acquired through post graduate diploma or equivalent experience or training plus
* Masters or equivalent level of experience of working at a senior level in specialist area.
* Evidence of post qualifying and continuing professional development
* Musthave an understanding of the background to and aims of current healthcare policy in BSOL and appreciate the implications of this on engagement
* Should have an appreciation of the relationship between the Department of Health, the CCG, Local Authoritiesand individual provider and commissioning organisations
* Working Knowledge of DOLS
* Working knowledge of the personaliation and SEND agenda
* Conversant with national and local guidance on NHS Continuing
* Health care/NHS Funded Care for Adultsfor Childrens Continuing Healthcare and with detailed understanding of the correct use of the national tools (checklist, DST, Fast Track)
* Understand the importance of measuring the quality of commissioned care
* Good understanding of Safeguarding issues and policy framework
* Knowledge of transition process for children and young people who are transitioning into Adults
*Can demonstrate a clear understanding of issues surrounding equality and diversity
* Understanding of the patient

* Conversant with Mental Health Act
* Able to investigate complaints and presentation to the Courts
* Working knowledgeable of Who Pays Guidance 2020
* Strong understanding of the
* operational management and
* clinical role of the NHS.
* Full understanding of Social
* Services and other Local Authority functions.* Knowledge & understanding of the current NHS agenda and of
* primary care.
* Highly developed communication skills both verbal & written and able to present complex cases to legal.
* Ability to communicate with clinical and all levels of staff effectively.
* Capability to work across boundaries and foster close collaboration; using influencing and negotiation skills.
* Ability to build effective working
* relationships across a range of
* health care and other organisations.
* Extensive nursing skills in Adults with complex healthcare need
* Excellent report writing skills, demonstrating sound clinical knowledge, with clear and succinct rationale.
* Excellent interpersonal/communication skills.
* Ability to demonstrate sensitive management of client and family/carer expectations.
* Demonstrated capabilities to manage own workload effectively and makeinformed decisions in the absence of required information, working to tight and often changing timescales to meet agreed actions and outcomes
* Experience of effective partnership working with other organisations at a senior level.

Desirable

* Specialist knowledge or experience to master's level equivalent
