Birmingham, West Midlands (commutable from Solihull, Coventry, Redditch, Walsall, Wolverhampton, Worcester, Tamworth)My Client are an NHS Trust based in West Midlands and are seeking a Senior Quality Nurse Assessor on an initial 6 month contract basis (37.5 Hours) Paying £286 per day (Inside IR35).

Job Summary

The post holder will support the CHC Business andClinical Leads in discharging their responsibility to provide assurance to all stakeholders that the CHC service are deliveringa high quality, effective, person centered and value for money service.
The post holder will support and undertake the monitoring of a range of quality and governance processes these include participating in the clinical verification of Fast Tracks, Checklists, Funded Nursing Care, and CHC assessments to ensure these are in line and appropriate with the NHS National Framework forContinuing Healthcare and Continuing Care.
The post holder will support the Continuing Healthcare team to meet its statutoryobligation of 28 days and receives the correct referral at first entry point into CHC.
Awareness of responsible commissioningguidance, and the varying different pathways of funded these inclusive of the Section 75 agreements with the LA including Care Act Assessment, SEND and Transition, Education and Mental Health and Learning Disabilities.

* To support the seamless implementation of the National Framework for Continuing Healthcare including contribution to the development of training programmes anddelivery plans as required to facilitate others learning.
* Within Birmingham and Solihull Clinical Commissioning Group provide advice, screen and co-ordinate assessments against the National Framework for Continuing Healthcare and Funded Nursing Care within Nursing Homes.
* To work with the Senior Nurses to process Funded Nursing Care and Continuing NHS Health Care Applications. Influence best practice by providing professional nursing advice on a range of physical and mental health nursing issues, which will require the individual to work in collaboration with partners and other professionals to ensure patients' health care needs are met 3 appropriately and effectively.
* To work closely with the Funded Nursing Care Lead Nurse by assisting in observing the quality of care in homes visited, identifying unmet need and being proactive in the encouragement of improvement in carestandards.
* A strong business focus as well as clinical expertise is a key requirement for this role to ensure delivery of best possible care within available funding representing "value for money".

The role will support the delivery of the CCGs statutory obligations in respect of CHC delivery, as well as supporting assist the delivery of the QIPP programme.

Experience* Report writing
* CHC clinical experience in practice
* Experience of working within the NHS
* A good understanding ofthe health and social care environment and roles and responsibilities within it
* Must have an appreciation of the relationship between the Department of Health, the CCG, Local Authorities and individual provider and commissioning organisations
* Experience of negotiating and liaising with multi-disciplinary team.
* Evidence of undertaking audit and implementing outcomes in arange of settings
* Ability to demonstrate autonomous practice, initiating and facilitating evidence based practice
* Commitment to partnership working across education, health, and local authority settings
* Negotiation and conflict management skills and the ability to influence decisions.

Qualifications

* Qualification at degree level or equivalent experience or competencies
* Evidence of an extensive knowledge of the National Framework for NHS CHC/FNC(2018); CHC policy and processes whether through formal qualification or equivalent experience
* Extensive experience of the application of the National framework for NHS CHC/FNC in practice Nursing Qualification
* Evidence of ongoing CPD / learning
