Their services encompass various forms of support, including day services, residential group homes, respite care, community support, home sharing, rehabilitative training, and multidisciplinary assistance.
Overview of Role:
The Community Care Coordinator position is pivotal in enabling individuals to actively participate in their communities while ensuring that service delivery aligns with evolving needs. This role involves working within a multidisciplinary team to provide health and social care support to individuals with intellectual disabilities. The Community Care Coordinator will play a central role in adapting service models to encompass holistic care approaches, in line with Person Centred Planning, HIQA standards, and emerging service requirements.
Responsibilities:
* Assess, plan, and evaluate care for individuals within the service.
* Coordinate hospital transitions and ensure continuity of care.
* Build positive relationships with individuals, families, and multidisciplinary teams.
* Advocate for individuals' rights and community participation.
* Facilitate access to healthcare services and promote independence.
* Ensure compliance with regulations and safety protocols.
Requirements:
* Registered nurse with NMBI.
* Experience in intellectual disability care, preferably in the community.
* Leadership skills and ability to manage teams effectively.
* Excellent communication and IT skills.
* Positiveattitude towards supporting individuals with disabilities.
