Lead Complex Case Manager (Learning Disability)
Birmingham
Band 7 (Inside IR35)
Full Time: Mon - Fri (37.5hrs)
6 Month Contract

DUTIES AND RESPONSIBILITIES

* The candidate will work jointly with health, social care and independent sector colleagues to ensure care management processes are appropriate and at time may be asked to present assessments to the BSOL continuing care panels for ratification.
* To follow the process laid out in the National Framework for NHS ContinuingHealthcare; to include organisation, co-ordination and participation in MDT eligibility recommendations to be verified by CCG decision makers.
* The post holder will be responsible for complex clinical caseload comprising of; Assessments, undertaking ofreview (3/12 CHC review unscheduled review and Fast Tracks and implementation on Personal Health Budgets and ongoing Case Management of Patients funded by Continuing Healthcare (CHC), Mental Health & Learning Disabilities.
* Ensure assessments conducted are robust, transparent and multi-agency (where required) for NHS Continuing Healthcare/NHS-funded Nursing Care.
* Analysis of supporting documentation to establish critical history of the patient's relevant needs to produce a specialist nursing assessment.
* Complete new referrals for NHS Continuing Healthcare within 28 days as per the National Framework for NHS Continuing Healthcare.
* To undertake NHS-funded Nursing Care (FNC) determinations for service users who require ongoing nursing care in a care home with nursing, ensuring that patients are assessed within current guidelines and then reviewed after three months/annually in line with DoH framework throughout the locality area
* Managing a defined caseload ( Assessment, Reviews (FNC/ CHCFast Track, reviewing cases), Transition, DOLS, LPS, and SEND, and Ongoing complex case management, Implementation of PHB, CareAct Assessment). This list is not exhaustive
* Supporting delivery and implementation of those assessed for those requiring apersonal health budget (PHBs) for your defined case-load.
* Supporting delivery and implementation of DOLS/LPS for your defined case-load.
* Ensuring the clinical team are maintaining data systems in line with standard operating policy and procedures* Providing relevant and timely specialist advice and guidance on functional and information matters relating to the service* Coordinating and supporting the delivery of quality and assurance including core documentation, information and reporting
*Ensuring the principles and duties of safeguarding are holistically, consistently and contentiously applied
* The candidate will be responsible for ensuring their mandatory training is up-to-date
* Manage the daily enquiries to the CHC Service and anycase management issues that arise unexpectedly

Education/Training/Qualifications

* Educated to degree level or equivalent level of experience working in a specialist area
* Dual registered sick children's nurse/Registered Nurse with a current NMCregistration or equivalent

Experience

* Previous experience of undertaking Continuing Care Assessments
* Significant experience of working with children and young people with complex and long-term health needs, disabilities and those with life limited/palliative or end of life
* Experience of negotiating and liaising with multi-disciplinary teams
* Evidence of undertaking audit and implementing outcomes in a range of settings
* Ability to demonstrate autonomous practice, initiating and facilitating evidence based practice
* Commitment to partnership working across education, health, and local authority settings
*Negotiation and conflict management skills and the ability to influence decisions.
* Evidence of leadership, management and team development
* Evidence of professional/clinical and managerial expertise and knowledge acquired through CPD

Knowledge* Working Knowledge of DOLS
* Working knowledge of the personaliation and SEND agenda
* Conversant with national and local guidance on NHS Continuing Health care/NHS Funded Care for Adults for Childrens Continuing Healthcare and with detailed understanding of the correct use of the national tools (checklist, DST, Fast Track)
* Understand the importance of measuring the quality of commissioned care
* Good understanding of Safeguarding issues and policy framework
* Knowledge of transition processfor children and young people who are transitioning into Adults
* Can demonstrate a clear understanding of issues surroundingequality and diversity
* Understanding of the patient experience agenda & its contribution to quality.
* Conversant with Mental Health Act 1983 and Care Act assessment
* Able to investigate complaints and presentation to the Courts
* Working knowledgeable of Who Pays Guidance 2020.
* Strong understanding of the operational management and clinical role of the NHS.
*Full understanding of Social Services and other Local Authority functions.
* Knowledge & understanding of the current NHSagenda and of primary care.
* Highly developed communication skills and able to present complex cases to legal.
* Ability to communicate with clinical and all levels of staff effectively.
* Capability to work across boundaries and foster close collaboration; using influencing and negotiation skills.
* Ability to build effective working relationships across a range of health care and other organisations.

Skills

* Extensive nursing skills in Adults with complex healthcare need
* Excellent report writing skills, demonstrating sound clinical knowledge, with clear and succinct rationale.
* Ability to demonstrate sensitive management of client and family/carer expectations.
* Demonstrated capabilities to manage own workload effectively and make informed decisions in the absence of required information.

Communication Skills

* Strong verbal and written communication skills
* Ability to construct papers and deliver clear ideas and concepts concisely and accurately to a diverse range of stakeholders and audiences
* Good presentation skills for conveying complex information and concepts#

Analytical

* Ability to analyse and interpret quantitative and qualitative data, using this to draw robust conclusions and recommendations
* Ability to identify risks anticipate issues and create solutions to resolve problems in relation to projects or service deliveryEquality & Diversity

* Ability to challenge and influence a range of internal and external stakeholders

Travel* Car driver/owner or reasonable alternative - able to meet the travel requirements of the post
