Job overviewTo be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. You will be responsible for a number of clinicalareas such as chronic disease management, infection prevention and control, health promotion, health prevention, well women andwell man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.
Experience
Essential
* Experience of working in a primary care environment
* Experience of working as a practice nurse orcommunity nurse
* Experience of chronic disease management
* Experience of infection prevention and control measuresDesirable
* Interest in Diabetes or COPD would be desirable
