My Client are an NHS Trust based in West Midlands and are seeking a Senior CHC Nurse Assessor - Personal Health Budgeton an initial 6 month contract basis (37.5 Hours) Paying £315 per day (Inside IR35).

Job Summary

The post holder will undertake comprehensive professional Continuing Healthcare and Funded Nurse Care assessments for adults over 18 years of age with complex health needs including the assessment, review and monitoring of care for those whose health needs are funded via NHS Continuing Healthcare within the dedicated time scales. This will include retrospective and appeal cases.

The post holder will act as a Case Manager for individual clients within Birmingham Solihull CCG who are receiving continuing health care funding to assess the needs of the client and their family to facilitate, plan and evaluate service provision to ensure the client achieves an optimum level of function and receives appropriate levels of care in the most suitable environment.

Experience

* Previous experience of undertaking Continuing Care Assessments
* Significant experience of working with children and young people with complex and long-term health needs, disabilities and those with life limited/palliative or end of life
* Experience of negotiating and liaising with multi-disciplinary team.
* Evidence of undertaking audit and implementing outcomes in a range of settings Ability to demonstrate autonomous practice, initiating and facilitating evidence based practice
* Commitment to partnership working across education, health, and local authority settings
* Negotiation and conflict management skills and the ability to influence decisions. Evidence of leadership, management and team development Evidence of professional/clinical and managerial expertise and knowledge acquired through CPD

Knowledge

* High level of clinical governance framework
* Working knowledge of the personalisation and SEND agenda
* Conversant with national and local guidance on NHS Continuing
* Health care/NHS Funded Care for Adults for Children's Continuing Healthcare and with detailed understanding of the correct use of the nationaltools (checklist, DST, Fast Track)
* Understand the importance of measuring the quality of commissioned care
* Good understanding of Safeguarding issues and policy framework
* Knowledge of transition process for children and young people who are transitioning into Adults
* Can demonstrate a clear understanding of issues surrounding equality and diversity
* Understandingof the patient experience agenda & it contribution to quality.
* Conversant with Mental Health Act 1983 and Care Act assessment
* Able to investigate complaints and presentation to the Courts
* Working knowledgeable of Who Pays Guidance 2020.
*Strong understanding of the operational management and clinical role of the NHS.
* Full understanding of Social Services andother Local Authority functions.
* Knowledge & understanding of the current NHS agenda and of primary care.
* Highly developed communication skills both verbal & written and able to present complex cases to legal.
* Working Knowledge of DOLS* Knowledge of Children's Commissioning arrangements
* Ability to communicate with clinical and all levels of staff effectively.
* Capability to work across boundaries and foster close collaboration; using influencing and negotiation skills.
* Ability to build effective working relationships across a range of health care and other organisations.

Qualifications

* Educated to degree level or equivalent level of experience working in a specialist area
* Dual registered sick children's nurse/Registered Nurse with a current NMC registration or equivalent
