My Client are an NHS Trust based in Lincolnshire and are seeking a CHC Case Manager on an initial 3 month contract basis (37.5 Hours) Paying£192 per day (Inside IR35).
Located at Cross O Cliff, however the Candidate will travel across Lincolnshire for face to faceassessments
Job Summary
* The post holder will work as a member of the NHS Lincolnshire Clinical Commissioning Group Continuing Healthcare Team.
* The CHC Nurse Practitioner will ensure that the care needs for Continuing Heathcare (CHC) are identified and met in accordance with the National Framework.
* The CHC Nurse Practitioner will ensure that the care needs for Continuing Heathcare (CHC) are identified and met in accordance with the National Framework.
* The post holder will support the CHC leads discharging their responsibility to provide assurance to all stakeholders that the CHC team are delivering services which are high quality, effective, person centred and value for money..
* The post holder will ensure that they adhere at all times to the relevant National Framework and guidance, NMC Code of Professional Conduct and work within established policies, procedures, protocols and systems ensuring quality standards and performance as directed by the Locality Manager or Senior Quality Officer
Role Requirements
* Registered Nurse RGN, RMN or RLDN
* 2 years post registration
* Significant knowledge of the continuing care framework and its application
* Good Knowledge of NHS and social services guidance
* Strong knowledge basearound safeguarding children and Adults
* Evidence of teaching/ mentorship
* Clinical Knowledge base in order to ensure comprehensive holistic assessment for continuing care.
* A background that can demonstrate clinical experience in a healthcare setting including inter-agency and mulita-agency.
* Evidence of continued professional development.
