My Client with an office based in Derby are seeking a Discharge to Assess Project Lead on a 6 month contract basis with (15 hours a week, flexible working) paying £321 per day
The role is currently remote, howevera successful candidate will be expected to work from an ICB base at least once per week. This could be either in the Derby location or the Chesterfield (Scarsdale) location.
Job Description
* Leading implementation of identified system D2A improvement initiatives
* Working with cross system partners to deliver pilots using PDSA methodology
* Leading/ participating in task and finish groups that will identify improvements to the trusted assessor process
* Reviewing system delivery against SOPsand identifying opportunities for improvement
* Facilitating development of processes that will enable acute and community clinicians to communicate more effectively with each other to discuss discharge
Skills and experience
* Experience workingin discharge/ flow strongly preferred
* Strong project management skills
* Strong transformational change background and proven ability to manage and lead impactful programmes over the short term
* Experience of working with cross system partners andacross organisational boundaries strongly preferred
