Lead Transition Nurse Band 7 (Hybrid)
£230 per day (Inside IR35)
Birmingham, West Midlands (commutable from Solihull, Coventry, Redditch, Wolverhampton, Walsall, Worcester, Tamworth)

My Client are an NHS Trust based inBirmingham and are seeking a Lead Transition Nurse on an initial 3 month contract basis (37.5 Hours) Paying £230 per day (Inside IR35).

The role is Hybrid and successful candidates will be required to come to the office at least one day a week.

KeyResponsibilities:

* To complete evidence-based assessment for continuing care provision in line with the National Frameworkfor Children's Continuing Care and local continuing care policy, using the Decision Support Tool, and drawing on the advice ofother professionals, whilst taking into consideration:
* The preferences of the child or young person and their family;
* Aholistic assessment of the needs of the child or young person and their family;
* Reports and risk assessments from multidisciplinary teams or evidence collated during the Education, Health and Care (EHC) plan assessment.
* To advise other professionals in identifying a child's need for continuing care provision (pre-assessment), determine whether or not a full assessment is necessary and to make referrals appropriately.
* To make recommendations to the CYP Continuing Care Eligibility Panel as to whether or not the child or young person has continuing care needs, ensuring that the continuing care process is (and is seen to be)fair, consistent, transparent, culturally sensitive, and non-discriminatory.
* To navigate children and parents through the local continuing care assessment process, providing advice and managing parents' expectations; liaising with families in challenging and complex situations.
* To attend Eligibility and Funding Panels, dispute resolution meetings, chair care review meetings and attend other multi-disciplinary team meetings.
* Through the work of the team ensure that CYP with complex mental healthneeds can access appropriate support to prevent escalation and ensure step down from crisis and or in patients' settings is expedited, including the development of provider led S75.
* Complete at least yearly, but dependent on clinical need formal reviews of care packages in conjunction with the ICB to ensure care provided is appropriate to needs.
* Lead on reviews jointly with Social Care and Education professionals where appropriate and should also inform the Education Health and Care Plan (EHCP) andSEND process.

Qualifications:

* Educated to degree level or equivalent level of experience working in a specialist area* Dual registered sick children's nurse/Registered Nurse with a current NMC registration or equivalent
