Birmingham, West Midlands (commutable from Solihull, Coventry, Redditch, Walsall, Wolverhampton, Worcester, Tamworth)

My Client are an NHS Trust based in West Midlands and are seeking a Locality Assessment and Review Manager on an initial 6 month contract basis (37.5 Hours) Paying £454 per day (Inside IR35).

Birmingham and Solihull (BSOL) Children, Young People and Learning Disability patient care pathways including section 75, SEND agenda, DoLS / LPS, Personal Health Budgets and Educational Health Care Plan.

The post holder will be required to work at any establishment at any time throughout the duration of their contract, normally within the location of the Organisation, or as set out under the terms of their contract

Job Summary

* The post holder will be responsible for the day-to-day operational management of the Birmingham Solihull CCG.
* The post holder will be providing excellent management and professional leadership to the Continuing Healthcare team.
* The postholder will lead the Clinical Team to ensure that all assessments and reviews and Fast Track are carried out in a timely mannerand to a high-quality standard.
* The post holder is responsible for ensuring the statutory requirements as laid out in the National Framework for NHS Continuing Healthcare (CHC) and NHS - Funded Nursing Care (FNC) is implemented.
* The postholder will develop robust processes and procedures and policies which are compliant with legislative and national guidance and in accordance with CCG responsibilities.
* The postholder is responsible for meeting the national and local KPI's and reporting directlyto the All Age Continuing Health Care Adults Lead. The post will report information on any delays which occur in meeting national framework guidance. This will include delays to assessments and reviews. Where necessary contribute to improvement strategy to prevent delays through quality issues in submission of DSTs to MDT.
* The post holder will be required to work in partnership with the Local Authorities in BSOL , Acute Trusts and Care Homes in ensuring timely discharges. Will be responsible for assisting and supporting the development of discharge pathways in relation to CHC and ensure the achievement of the Local Area Agreement target in relation to delayed transfers of care is met.
* The post holder is responsible for ensuring all decisions made about CHC and their rationale should be transparent and communicated clearly in writing to the relevant bodies and individuals ina timely manner and within guidance set out the national framework.

Education and Knowledge

* Demonstrable experience ofworking in a large complex organisation.
* Experience of working within the NHS.
* Masters or equivalent level of experienceof working at a senior level in specialist area.
* Evidence of post qualifying and continuing professional development
* Must have an understanding of the background to and aims of current healthcare policy in BSOL and appreciate the implications of this on engagement
* Should have an appreciation of the relationship between the Department of Health, the CCG, Local Authorities and individual provider and commissioning organisations
* Working Knowledge of DOLS
* Working knowledge of the personalisation and SEND agenda
* Conversant with national and local guidance on NHS Continuing Health Care/ NHS Funded Care for Adultsand Children's Continuing Healthcare and with detailed understanding of the correct use of the national tools (checklist, DST, Fast Track)
* Understand the importance of measuring the quality of commissioned care
* Good understanding of Safeguarding issues and policy framework
* Knowledge of transition process for children and young people who are transitioning into Adults* Can demonstrate a clear understanding of issues surrounding equality and diversity
* Understanding of the patient experienceagenda & its contribution to quality.
* Conversant with Mental Health Act 1983 and Care Act assessment
* Able to investigate complaints and presentation to the Courts
* Working knowledgeable of Who Pays Guidance 2020.
* Strong understanding ofthe operational management and clinical role of the NHS.
* Full understanding of Social Services and other Local Authority functions.
* Knowledge & understanding of the current NHS agenda and of primary care.
* Highly developed communication skills both verbal & written and able to present complex cases to legal.
* Ability to communicate with clinical and all levels of staff effectively.
* Capability to work across boundaries and foster close collaboration; using influencing and negotiation skills.
* Ability to build effective working relationships across a range of health care and other organisations.
* Extensive nursing skills in Adults with complex healthcare need Excellent report writing skills, demonstrating sound clinical knowledge, with clear and succinct rationale.
* Excellent interpersonal/communication skills.
* Ability to demonstrate sensitive management of client and family/carer expectations.
* Demonstrated capabilities to manage own workload effectively and make informed decisions in the absence of required information, working to tight and often changing timescales to meet agreed actions andoutcomes
* Experience of effective partnership working with other organisations at a senior level.
