My Client are an NHS Trust based in West Midlands and are seeking a Senior CHC Nurse Assessor who specialises in ABI onan initial 6 month contract basis (37.5 Hours) Paying £315 per day (Inside IR35).

Job Summary

* The post holder will work as part of a dynamic team, including commissioners and providers in delivering an effective service supporting the effective placement and quality assurance of individuals requiring Specialist Placements or ABI provision.
* The post holder will possesssound knowledge and understanding of the relevant guidance and frameworks and Commissioning approval systems related to Specialist Placements and ABI.
* The post holder will be expected to communicate clearly and knowledgeably in relation to all aspectsof ABI, Specialist Placements relating to ABI ensuring people are placed appropriately, ensuring continues focus on the individual and their needs, and facilitating the most appropriate and cost-effective pathway for individual care.
* The post holder,with support from the Senior Clinical Leads and commissioned providers will support the management, co-ordination and monitoringof individuals requiring Specialist Placement Services. It will be necessary to work closely with patients and their relatives,Local Authorities, and commissioned providers to ensure timely and appropriate assessments, care planning and reviews, and to ensure effective discharge planning is undertaken.
* With the support of the Senior Clinical/ Professional Lead, the post holder will also be required to actively participate and offer constructive advice and guidance and reports relating to any review process, often involving very complex cases, including the Court of Appeal and attend and participate in additional meetings relating to such a process.

Main duties and responsibilities

* The post holder will have a flexible approach to care and willwork across inter-agency boundaries, maintaining good communication and liaison skills with all stakeholders.
* The post holder will support the Lead Complex Manager and Commissioners to ensure that their programme of work is planned and managed effectively and take the lead in assisting in their successful delivery, including the compilation of reports and data summaries.
* The post holder must be clinically or professionally qualified and under the supervision of the Senior Clinical/ Professional Leadwill be responsible for:
* Co-ordinate timely Quality Assurance reviews and visits for patients who are receiving, SpecialistPlacements and ABI care provision.
* Act as a specialist practitioner for a specific area, including interpretation and implementation of relevant national guidance.
* Under the supervision of the Senior Clinical/ Professional Lead and Senior Commissioners, lead on the management of contentious cases which are highly complex and sensitive, acting as CCG representation in legalproceedings.
* Ensure relevant NHS guidance is followed and that services are delivered to maximise patient safety by meetingtheir assessed on-going clinical needs.
* Facilitate and contribute to multi-disciplinary assessments and patient reviews inline with standard operating procedures.
* Liaise and actively contribute with appropriate functions within the CCG, includingthe delivery of complex cases and the Local Authorities across the ICS, and other CCGs.
* Manage competing demands to deliveron difficult and complex tasks/issues within agreed timeframes.
* With support from the Senior Clinical/ Professional Lead, lead and implement Liberty Protection Safeguards for appropriate patients in the Community with legal team support.
* Ensure Personal Health Budgets are promoted by the multi-disciplinary team and ensure robust systems are set up for implementing these byliaising with internal staff (Finance and commissioning) community case managers and individuals / families to establish these.* Take a leadership role in promoting and safeguarding the welfare of vulnerable adults.
* Represent the CCG and alongsideof the Safeguarding Lead influence the local safeguarding processes, ensuring safeguarding plans are reviewed and robustly monitored for people requiring Specialist Placement Service.
* Work with the Senior Clinical/ Professional Lead to manage the team's business support function, including developing and managing the team's business plan, progress and reporting risk and issue management, achieving quality outcomes.
* The job description and person specification may be reviewed on an ongoing basis in accordance with the changing needs of the department and the Clinical Commissioning Group.

Experience

Essential

* Report writing
* Demonstrable experience of effective team management
* CHC clinical experience
* Experience of working withinthe NHS
* A good understanding of the health and social care environment and roles and responsibilities within it
* Must have an appreciation of the relationship between the Department of Health, the CCG, Local Authorities and individual provider and commissioning organisations

* Good working knowledge ofCHC processes and procedures is essential
* Clear communicator with excellent written and presentation skills; capable of constructing and delivering clear ideas and concepts concisely and accurately to a diverse and varied range of audiences consistingof internal and external stakeholders.
* Proven interpersonal skills to work with staff at all levels as well as individualsand/or their representatives.
* Ability to prepare and produce concise yet insightful communications for dissemination to senior stakeholders and a broad range of stakeholders as required.
* Experience of creating and giving presentations to a varied group of internal and external stakeholders.
* Ability to analyse very complex issues where material is conflicting and drawn from multiple sources.
* Demonstrated capability to act upon incomplete information, using experience to make inferences and decision making.
* Numerate and able to understand complex financial issues combined with deep analytical skills.
* Demonstrated capabilities to manage own workload and make informed decisions in the absence of required information, working to tight andoften changing timescales.
* Demonstrated capability to plan over short, medium, and long-term timeframes and adjust plans andresource requirements accordingly.
* Working collaboratively with a range of stakeholders and the management of variety of relationships
* Proven negotiations skills around the review of packages of care to ensure outcomes are delivered while ensuringbest use of resources.
* Resilience to deal with highly emotive and contentious issues, complaints, and confrontation.

Education

Essential

* Registered with NMC (or equivalent to specialist health profession) or Allied Health Professional (AHP) or Social Worker professional qualification.
* Educated to Post-graduate degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist area.

Desirable

* Master's Degree level or equivalent professional experience in specialist area and/or management.
