6 Month Contract
Birmingham, West Midlands (commutable from Solihull, Coventry, Redditch, Walsall, Wolverhampton, Worcester, Tamworth)
MyClient are an NHS Trust based in West Midlands and are seeking a Senior CHC Nurse Assessor on an initial 6 month contract basis(37.5 Hours) Paying £286 per day (Inside IR35).
Candidates must have PHB experience
Experience
* Previous experienceof undertaking Continuing Care Assessments
* Significant experience of working patients with complex and long-term health needs, disabilities and those with life limited/palliative or end of life
* Experience of negotiating and liaising with multi-disciplinary team.
* Evidence of undertaking audit and implementing outcomes in a range of settings
* Ability to demonstrate autonomous practice, initiating and facilitating evidence based practice
* Commitment to partnership working across education, health, and local authority settings
* Negotiation and conflict management skills and the ability to influence decisions.
* Evidence of leadership, management and team development
* Evidence of professional/clinical and managerial expertise and knowledge acquired through CPD
Qualifications
* Educated to degree level or equivalent level of experience working in a specialist area
* Registered Nurse with a current NMC registration or equivalent
